Quality care, clearly defined.
These terms explain how care will be provided by Chequers Health Centre and Chequers Health Group Ltd. By attending for treatment, you agree to these terms. Updates may be made from time to time and will apply only to new episodes of care. You will be asked to accept any revised terms before they take effect. 1. About Us
Chequers Health Group Ltd. is a company registered in England and Wales. Company registration number: 12653799. Registered office: Chequers Health Centre, Rushlake Drive, Bolton, Greater Manchester, BL1 3RL. The services described in these terms are provided in England and are governed by English law. We are regulated by the CQC and provide Diagnostic and Screening Procedures, Maternity and Midwifery Services, Family Planning, Surgical Procedures, and Treatment of Disease, Disorder, and Injury.
2. Insured Patients
You agree to pay for your procedure. Where you have private medical insurance, we will where possible process the insurance claim on your behalf, provided you have given us and your insurer all information required to do so. If this information is incomplete or inaccurate, we may be unable to process your claim and you will be required to pay directly. If your insurer fails to settle our invoice within 30 days of the date of issue, we will assume the outstanding amount will not be paid and will invoice you directly. It is your responsibility to confirm with your insurer in advance that your care is covered. Please note that some insurers apply care guidelines that may differ from our consultants' medical opinion, which may affect what elements of your care are covered.
3. Self-Pay: Fixed Price
Where your procedure letter confirms that you have been offered a Fixed Price, a confirmation email and disclaimer setting out the fixed price details will be sent to you. Payment in full is required prior to your appointment. Appointments are confirmed only once payment is received. Only credit or debit cards are accepted; cash is not accepted.
Unless your disclaimer states otherwise, your Fixed Price includes: all consultant fees; nursing and post-discharge care such as removal of stitches; minor surgery room charges; pre-admission fitness tests; all tests, treatments, and dressings required during your visit; your essential medical or surgical equipment; one clinical follow-up appointment if required; all clinically necessary post-discharge pathology; treatment for any clinical complications arising from your procedure; and medications and prescriptions.
The Fixed Price does not include: your initial consultation; care not listed in your procedure package; care unrelated to your procedure; or care received at a location other than your booked site.
4. Self-Pay: Cancellations and Complications
If you decide not to proceed with your procedure, you will be charged for any care received up to the point of cancellation at Standard Rates, but no more than the Fixed Price for included items. A cancellation fee may apply if you cancel within 48 hours of a scheduled appointment. Refunds will be made only to the original cardholder; cash refunds are not issued.
While the organisation and your consultants will take every care to ensure a satisfactory outcome, no clinical procedure is entirely risk-free and results cannot be guaranteed. Your Fixed Price includes the cost of treating any clinical complications identified by your consultant as arising directly from your procedure, provided you have followed the advice of your clinical team. Where treatment for complications cannot be provided, appropriate follow-up care may be provided by your GP or the NHS.
5. General Terms
Consent and capacity: by attending for treatment, you confirm that you have been given sufficient information about your procedure and its risks and alternatives, that you are consenting voluntarily, and that you have full capacity. You will be asked to sign a consent form on the day of your procedure.
Behaviour and conduct: the organisation reserves the right to refuse or discontinue treatment if a patient's behaviour towards staff or other patients is abusive, threatening, or otherwise inappropriate. In such cases, no refund will be issued for care already received.
Medical records: the organisation maintains records of your care in accordance with its legal and regulatory obligations. Please refer to the Patient Privacy Notice (Chapter 7) for details of how your personal and health data is handled.
Changes to services: the organisation reserves the right to make changes to services where required by applicable law, clinical guidelines, or regulatory requirements. If a change affects your care, you will be contacted to explain the consequences.
Governing law: these terms are governed by and construed in accordance with English law. The courts of England shall have non-exclusive jurisdiction over any disputes. Residents of Scotland or Northern Ireland may also bring proceedings in their respective courts.
6. Cancellations and Refunds
You have 14 days from the date of booking to exercise your right to change your mind, provided no treatment has started. You can exercise this right by writing to [email protected]. If treatment has begun, any refund will reflect the cost of treatment already received. Where the organisation cancels your appointment, a rescheduled appointment or full refund will be offered. All approved refunds will be processed within 14 days by the original payment method.
7. Limitation of Liability
The organisation is not liable to you for any loss of profit, loss of business, or business interruption. The organisation's total liability to you in connection with any service is limited to the total price paid for that service. Nothing in these terms excludes or limits any rights or remedies you may have under the Consumer Rights Act 2015 or any other applicable UK consumer protection legislation that cannot lawfully be excluded.
